SOAP with Animalog


Clinical notes are a vital part of any pet treatment centre. They allow you to track all treatments given to patients so you can see exactly what is required ongoing, and allow you to communicate the outcomes of your treatment to any treatment partners such as the patient’s veterinary practice.

The recommended method of standardising your clinical notes is SOAP. If you are unfamiliar with SOAP it’s a mnemonic which stands for Subjective, Objective, Assessment and Plan.

Subjective includes only the information from the client – this could be a summary or direct quote for example.

The Objective section is any data or information that agrees with the subjective.

The Assessment is your professional and clinical option based on the subjective and objective statements provided.

And then the Plan states any future clinical work, any referrals/recommendations made, and of course, any follow-up treatments needed.
